- Space Application Form
- Letter of Intent
- Merchant Information Sheet (Corporation or Sole Proprietorship)
- Annex C (Food or Non-Food)
- Franchise Agreement*
- Endorsement Letter from Franchisor*
- Certificate of Registration with SEC or DTI
- Articles of Incorporation and By Laws
- Corporate Secretary’s certificate affirming authority of contract signatory
* if merchant is coming in as a franchisee
